Community Life Program
About Community Life
The Story County Community Life Program is a community based human service agency, which assists persons with disabilities to live, learn, work, and socialize in their environments of choice. The Community Life Program utilizes a person centered approach to service delivery, which recognizes the unique needs and abilities of all persons served.

Vision Statement
"To be the premier community resource for persons desiring to live beyond their disability(s).”

A Participant Centered Approach
Each participant in the program is provided with the opportunity to create an individualized program plan, which will enable them to achieve goals that are based on what is important to them. In order to assist a person to be successful and satisfied in a chosen environment we focus on intervention that develop skills, and supports needed in a particular environment.

Inherent in this person's centered approach to service are the core values of increased functioning, success, satisfaction, choice, outcome orientation, support, and growth potential.
