Systems with Discharge Ports
Systems that have discharge ports include, but are not limited to: individual mechanical / aerobic systems, multiple-pass filtration systems (e.g., recirculating sand filters, recirculating textile filters, and recirculating foam filters), aquatic systems (e.g., constructed wetlands and waste stabilization ponds), free-access sand filters, and single-pass and packed-bed filtration systems (subsurface sand filters, peat filters, and other single-pass packed-bed filters).

Requesting a Permit
The National Pollutant Discharge Elimination System (NPDES) form should be filled out and sent to the address listed at the bottom of the form. The Iowa Department of Natural Resources (IDNR) will notify the applicant and the Story County Health Department that a permit has been issued and give the number assigned to the new discharge permit. Under the terms of NPDES #4 the owner / permit holder is required to follow the requirements as set forth by the IDNR. These requirements can be found in a document called "NPDES Requirements."
