Overview
Story County Case Management is accredited by the Department of Human Services as a case management provider. Case Management provides support and advocacy to people with chronic mental illness, intellectual disabilities, developmental disabilities, and brain injury, with a focus on individual strengths and desires.

Assistance Provided
Story County Case Management offers a progressive approach to assist people with the following:

• Accessing and securing funding
• Enhancing natural supports
• Identifying needs and desires
• Linking to community resources
• Monitoring and coordinating services
• Referring to appropriate services
