Scheduling routine optometry and audiology appointments
You may now schedule routine optometry and audiology appointments without a referral from your Primary Care Provider. You must be registered in the VA and eligible for care, but you do not need to have an assigned Primary Care Physician to coordinate your routine eye and ear care.

You may use My HealtheVet Secure Messaging to request an optometry or audiology appointment.  Just select the appropriate group using the drop down menu. Please specify the date and time you would prefer to be seen, and if you are willing to be scheduled for the first open appointment.

You may also schedule by telephone.

To schedule an audiology appointment, call 515-699-5609 or contact the nearest Community-Based Outpatient Clinic (CBOC).

To schedule an optometry appointment call 515-699-5815 or 1-800-294-8387 option 2, then option 5.
You may also call your nearest CBOC or schedule an appointment when you check-in/out at your next Primary Care visit.

WASHINGTON (July 6, 2016)- The Department of Veterans Affairs Eliminates Paper Signature Requirements for Veterans Enrolling in VA Health Care.

The Department of Veterans Affairs (VA) announced today it has eliminated paper signature requirements for Veterans wishing to enroll in VA health care.  Effective immediately, VA has amended its enrollment regulations to allow Veterans to complete enrollment applications for enrollment in VA health care by telephone without the need for a paper signature. This action also accelerates VA’s effort to enroll all Combat Veterans with pending enrollments as part of its ongoing Veterans Enrollment Rework Project (VERP).

By adding this telephone application option to VA’s regulations with this amendment, VA will now offer three ways to enroll under 38 CFR 17.36(d) (1). This option provides Veterans a convenient third enrollment option in addition to the paper VA Form 10-10 EZ and the online health care application.
